Our company’s unique approach addresses the total well-being of our clients and their families through a comprehensive program that includes assessment, planning, service coordination, advocacy, and direct care by our well-trained care team – led by a quality of care manager. Without the type of care offered by Home Health Care Plus, people with chronic conditions would be placed in nursing homes, dementia units or other types of long-term care institutions.

We believe this is true because our work at every level is based on delivering quality personal care. By empowering our clients to live a safe, healthy and independent lifestyle, the Home Health Care Plus clinical team of caregivers, along with the families and professionals with whom we collaborate, create a culture of awareness, shared concern about the well-being of those we serve and peace of mind knowing you or your loved ones are safe.

We encourage you to contact our local office or initiate a conversation through this website. It can be the beginning of a healthier life for you or a person you care about.

Our Process

Successful outcomes. The process of ensuring that clients can live at home safely and in the best state of health possible begins with careful planning. Here is an overview of the Home Health Care Plus approach to assessing your individual situation and arriving at a plan that will work for everyone involved.

Every person needs to be understood within the context of their environment and personal history. They arrive at their level of care need in a way that is uniquely their own. Our Home Health Care Plus licensed, clinical professionals conduct an in-depth assessment of each individual’s needs, preferences, strengths, support systems and resources in consultation with all concerned parties. Our professionals listen and learn before they respond with solutions and services. The input we receive enables us to understand all of the clinical and psychosocial factors that will influence the customization of the care plan.

When all input and information is collected and reviewed, your Home Health Care Plus team will present a recommended care plan that is built around the needs and requirements of the client and his or her support system. We will provide details of the caregiver, the schedule and timing, the frequency, intensity, and cost. This plan will be shared with and approved by all involved parties.

The circumstances surrounding a person’s level of need may change suddenly or gradually. As they do, Home Health Care Plus will be there at any hour of the day or night to provide the level of care and support necessary, as well as to recommend appropriate and ongoing changes to the care plan.

The Home Health Care Plus Team

Home Health Care Plus caregivers are defined by a set of very important criteria. They are employees and caregivers united by the common understanding that the person they help is a beloved individual, a cherished companion, and a person who deserves to be treated with dignity and respect.

An expert Quality Care Manager is assigned to the supervision and care coordination of the delivery of all aspects of client care. The Quality Care Manager can be a nurse or scheduling coordinator depending on the nature of the client’s needs. This is the point person coordinating all the care received by each client.

The Quality Care Manager oversees the Care Plan for each client, including both long and short-term care, based on an integrated home assessment made at the client’s home. They schedule the caregiver, resolve scheduling conflicts, make in-home functional assessments, coordinate additional services needed such as transportation, social stimulation, counseling, and in-service training for the maintenance of a safe environment.

Any questions that the family or physicians have about the care can be addressed to the Quality Care Manager. Responsibilities also include providing advice and coordination and collaboration with professionals and outside providers. They also identify changes in the client’s condition and recommend when a new team member with a separate specialty needs to be involved in the case.

Home Health Care Plus Licensed Nurses perform duties that include treating and educating clients about various medical conditions and providing advice and support to clients, family members and physicians.

Home Health Care Plus therapists help people function the best they can in their environment. They provide comprehensive services/treatment in accordance with the care plan and supervise the supportive caregiver to enhance the overall quality of care provided.

These professionals assess the client’s diet and engage the client in regular physical and cognitive activities adapted to their level of functioning.
